In lieu of an abstract, here is a brief excerpt of the content:The Ethics of Surgical Interventions for Body Integrity Identity Disorder and Gender DysphoriaNicanor Pier Giorgio Austriaco, O.P.IntroductionOn May 20, 2009, Fox News featured a report that described the life of a man named "John" who had spent his life struggling with Body Integrity Identity Disorder (BIID).1 In a phone interview, John admitted that he remembers wanting to amputate his leg when he was between seven and eleven years of (...) age, by sticking his leg under the rear wheel of a bus. He never went through with it but continued to experience an intense desire to amputate his healthy limb for the following fifty-plus years. Described for the first time in 1977 as apotemnophilia, BIID was originally thought to be a psychological disorder characterized by sexual arousal associated with the desire to be an amputee. However, it was renamed when Dr. Michael First, a professor of clinical psychiatry at Columbia University, proposed that the primary motivation behind the condition was an issue of identity rather than of sexual desire.2 This proposal is controversial.3 Despite much [End Page 1003] debate, BIID was not included in the most recent edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), which was published on May 18, 2013.In this essay, we will consider the ethics of surgical interventions for BIID and Gender Dysphoria (GD).4 We begin with a discussion of the principle of totality that distinguishes ethical from unethical modifications of the body. We will refer specifically to the latter category of surgical interventions as "mutilations," though the Catholic moral tradition's use of the term is itself ambiguous.5 This is followed by an extensive moral discussion of (1) amputations of healthy limbs for patients struggling with BIID and (2) sex reassignment surgeries for those who identify themselves as transgender persons. I will propose that an ethical argument can be made to justify the former but not the latter.The Principle of TotalityBecause of the goodness of the human body, its integrity is a good that has to be protected and preserved. The ethical principle that is used to judge the morality of bodily interventions that impinge upon this bodily integrity is the principle of totality.6 It was articulated by St. Thomas Aquinas in his Summa theologiae as follows:Since a member is part of the whole human body, it is for the sake of the whole, as the imperfect for the perfect. Hence a member of the human body is to be disposed of according as it is expedient for the body.... If, however, the member be decayed and therefore a source of corruption to the whole body, then it is lawful with the consent [End Page 1004] of the owner of the member, to cut away the member for the welfare of the whole body.7Over the centuries, Catholic moral theologians appealed to the principle of totality to justify three different scenarios that involved the sacrifice of a bodily organ for the sake of the whole human organism. The first case involves a diseased organ that has to be removed to save the life of the soldier wounded on the battlefield—say a gangrenous, decaying limb as in the example of Aquinas. The second case involves the healthy foot of a man who is trapped on a railroad track that has to be amputated to save his life from a collision with a train, and the third case involves the healthy limb of an individual that has to be amputated in response to a tyrant who orders him to cut off his own hand or lose his head to the executioners instead. In these cases, the sacrifice of the part would be morally permitted as a necessary means of preserving the life of the individual. As Fr. Gerald Kelly, S.J., points out, this final case is particularly significant because it involves a healthy limb whose very presence—and not its diseased or trapped condition—becomes a source of vital harm for the organism.8How does one justify the principle of totality? What MacKinnon offers here is a provocative and original analysis of the meaning of the word "true." His applications are in the areas of statements in general, scientific theories, and theological propositions. One reason for the interest of the book can be found in MacKinnon’s intellectual odyssey. Setting out from a starting-point of standard neo-scholastic textbook philosophy and theology, MacKinnon has come to a highly personal synthesis to which he is willing at least tentatively to apply the label, "ontological pragmatism." Stages along the way—and elements in the synthesis—include Bernard Lonergan’s transcendental Thomism, Wilfrid Sellars’ neo-Kantian version of analytical epistemology, and much detailed work of MacKinnon’s own in analytical philosophy of science. The provocative character of Truth and Expression is likely to bring criticism of the author for misuse of his sources. Wittgensteinian philosophical therapists are likely to bemoan MacKinnon’s relapse into ontologism, even if he qualifies it as a "pragmatic" version. Sellars is likely to object to being lumped with Lonergan, and the latter would probably be upset with the uses to which his former student has put his insights. It may also be worthwhile here to express a demur or two with respect to MacKinnon’s use of his Thomistic sources, e.g., concerning his theory of meaning. It seems that MacKinnon has accepted too naively what might best be called "textbook Thomism." It is true that Aquinas offers a mentalist view, and true also that both Aristotle and Aquinas speak of words as signs of concepts. While this can give rise to a private-language mentalism of the kind Wittgenstein would criticize, it must be recalled that Aristotle and Aquinas were writing not only before Wittgenstein but also before Kant. Whatever can be said of Aquinas’ "mentalism," it is certainly true that in other contexts Aquinas has a social theory of learning that is, at least on the surface, incompatible with a private language theory. Again, with respect to theological truth it seems likely that MacKinnon has been misled by Thomistic manuals, at least when he claims that what Aquinas offers is "a geometrically ordered deductive system" in which "the primacy of Jesus as a revelation of God does not fit." Historical studies of the function of his Summa Theologiae in Aquinas’ scheme for theological education do not support the view of Aquinas as a theological deductive system builder. Nor is it true, as MacKinnon seems to hold, that Aquinas’ view of theological truth is more Aristotelian than biblical. Aside from these qualifications on his use of sources, MacKinnon’s book offers an abundance of critical thinking, analytical insight, and systematic ability. For instance, the reader interested in MacKinnon’s original approach to an adequate epistemology of science will here, in chapter three of Truth and Expression, find the best summary yet of his many contributions in that area. Whether or not, in the last analysis, MacKinnon’s synthesis will be historically significant remains to be seen. What seems likely now is that if it does, the ingredient elements of Lonergen, [[sic]] Sellars, Wittgenstein, Quine, etc., will be more muted, and MacKinnon’s own synthesis will stand on its own. Starting from the novel "O alienista" by Machado de Assis, I aim at discussing on the "normal" and the "pathological", referring to Georges Canguilhem and his work The normal and the pathological. Trying to understand how medicine states the concept of "normal" and, as opposed to it, the concept of pathological, Canguilhems thought was influencing a wide range of knowledges, going from the understanding of the individuals and the valorisation of the normativity to the whole of questions concerning the trinomial ethics-politics-psychology. In this paper, I reflect, from my practice as pathologist, on the sense of the morphological diagnosis and I question myself how a dipper knowledge of morphological mutations relates to a better understanding of pathological situations. I highlight the tension between the subjective and objective dimension deciding with uncertainty, as it is peculiar of the process that leads to diagnosis. Also, I support the necessity of the clinical reasoning to the reasonability of every diagnosis. (shrink)

Henri Lelevel’s La philosophie moderne par demandes et réponses is a very interesting as well as pretty neglected attempt to disseminate the new philosophy among a larger audience, including the non specialists. Either the style of presentation or the oversimplification of the topics discussed is clearly intended to reach people interested to a smattering of philosophy. More than the comparisons between the traditional and the new philosophy and the compendia, this work vouches for the great interest toward the new philosophy. (...) In this particular case the model is the philosophy of Malebranche, whose interpretation of Descartes thought is preferred by Lelevel, worried by the excessive twist given toward empiricism in P.-S. Régis rendering of Cartesian philosophical system. Lelevel’s logic is directed to eliminate the confusion between sense data and knowledge, echoing also Malebranche’s notion of éténdue idéale, one of the major contribution to the reconsideration of Descartes’ thought. This paper aims to sketch the mains topics discussed in the four sections of Lelevel’s work, with some cross-references to Malebranche’s writings. (shrink)

Drawing on Ciceronian rhetorical tropes, Thomas Aquinas treats the rite of the Eucharist in terms of the classical ars memoriae. The Eucharist, for Aquinas, is the schooling in desire whereby we are trained to order the associations of our memory to their proper objects in terms of their relations to God. He thus conceives of the liturgy of the Mass as rhetoric proper, which truly teaches, moves and delights. Since memory is the condition of all thought, as both Thomas and (...) The Summa Theologiae ranks among the greatest documents of the Christian Church, and is a landmark of medieval western thought. It is regularly consulted by scholars of theology, philosophy and a range of related academic disciplines. This paperback reissue of the classic Latin/English edition first published by the English Dominicans in the 1960s and 1970s has been undertaken in response to regular requests from around the world. The original text is unchanged, except for the correction of a small number (...) of typographical errors. The parallel English and Latin texts can be used successfully by anybody with a basic knowledge of Latin, while the presence of the Latin text allowed the translators a degree of freedom in adapting their English version for modern readers. Procedure manuals and textually mediated deathThe procedure manual as a document represents the practice of nursing care. Analysis of such manuals allows us to explore discourses of nursing and the ways in which they frame nursing practice. A critical analysis of a hospital procedure manual using discourse analysis was undertaken. A specific excerpt concerning ‘Last offices’ is used as an example of the institutionalisation of organisational values and beliefs as these influence nursing care. ‘Last offices’ directs nursing practices related to (...) the death of a patient, and the laying out as preparation for the funeral director or coroner. The analysis shows that procedure manuals reproduce hegemonic ideologies of nursing. The analysis also shows how nursing practices are constituted by intersecting forces of differing locations such as the hospital as an organisation, the healthcare system and the wider society within which these function. _Manual of Regulation-Focused Psychotherapy for Children with Externalizing Behaviors: A Psychodynamic Approach_ offers a new, short term psychotherapeutic approach to working dynamically with children who suffer from irritability, oppositional defiance and disruptiveness. _RFP-C_ enables clinicians to help by addressing and detailing how the child’s externalizing behaviors have meaning which they can convey to the child. Diminish the child’s use of aggression as the main coping device by allowing painful emotions to be mastered more effectively. Help to systematically address avoidance mechanisms, talking to the child about how their disruptive behavior helps them avoid painful emotions. Facilitate development of an awareness that painful emotions do not have to be so vigorously warded off, allowing the child to reach this implicit awareness within the relationship with the clinician, which can then be expanded to life situations at home and at school. This handbook is the first to provide a manualized, short-term dynamic approach to the externalizing behaviors of childhood, offering organizing framework and detailed descriptions of the processes involved in _RFP-C_. Supplying clinicians with a systematic individual psychotherapy as an alternative or complement to PMT, CBT and psychotropic medication, it also shifts focus away from simply helping parents manage their children’s misbehaviors. Significantly, the approach shows that clinical work with these children is compatible with understanding the children’s brain functioning, and posits that contemporary affect-oriented conceptualizations of defense mechanisms are theoretically similar to the neuroscience construct of implicit emotion regulation, promoting an interface between psychodynamics and contemporary academic psychiatry and psychology. These all require a richer semantic picture than standard truth values in one static environment. Pointing by apes is near-ubiquitous in captivity, yet rare in their natural habitats. This has implications for understanding both the ontogeny and heritability of pointing, conceived as a behavioral phenotype. The data suggest that the cognitive capacity for manual deixis was possessed by the last common ancestor of humans and the great apes. In this review, nonverbal reference is distinguished from symbolic reference. An operational definition of intentional communication is delineated, citing published or forthcoming examples for each of the defining (...) criteria from studies of manual gestures in apes. Claims that chimpanzees do not point amongst themselves or do not gesture declaratively are refuted with published examples. Links between pointing and cognitive milestones in other domains relating means to ends are discussed. Finally, an evolutionary scenario of pointing as an adaptation to changes in hominid development is briefly sketched. (shrink)

Cardinal Mercier’s Manual of Modern Scholastic Philosophy is a standard work, prepared at the Higher Institute of Philosophy, Louvain, mainly for the use of clerical students in Catholic Seminaries. Though undoubtedly elementary, it contains a clear, simple, and methodological exposition of the principles and problems of every department of philosophy, and its appeal is not to any particular class, but broadly human and universal. Vilfredo Pareto's Manual of Political Economy is a 'classic' study in the history of economic thought. It is not only one of the leading works in the Lausanne tradition of economics, which centres on the theory of general equilibrium, it is one of the most important books in the history of neoclassical economics. This 'critical edition' of Pareto's Manual of Political Economy is a very significant work for two main reasons. First, it is the only variorum translation of the 1906 (...) Italian language Manuale di Economia Politica and the 1909 French language Manuel d'Économie Politique. Second, it includes extensive contributions by the editors, all of whom are authorities on the work of Pareto.
